From MVP to AI platform

01 · Where It Started

Two surgeons, a spreadsheet, and a vision nobody else was building

Dr. Vinayak Rengan and Dr. Gopi Ramu are practising surgeons who sat the NEET Super Speciality exam themselves. They knew the problem first-hand: thousands of doctors — already qualified specialists — preparing for one of India's most demanding post-graduate entrance exams had nowhere to go. The few platforms that existed were built for NEET UG or NEET PG, and the limited players in the super-speciality space charged 3× more for a fraction of the content quality.

So they built Surgtest themselves. The early version was entirely manual — a spreadsheet for content tracking, WhatsApp groups for student queries, and offline meet-ups for doubt clearing. It worked. Students started getting top ranks. Word spread. But every new student meant more WhatsApp messages to answer, more spreadsheet rows to manage, more manual work that took time away from actually creating content.

"We had 200 students getting top ranks on WhatsApp and spreadsheets. Imagine what we could do with an actual platform."

Scaling beyond a few hundred students was impossible without a proper platform. Content couldn't be updated centrally. Student performance was invisible. Subscriptions were managed by hand. Marketing had no tools to reach students at the right time. And with NEET SS exam dates fixed on the calendar, demand didn't grow gradually — it spiked, crashed the system, and spiked again.

Dr. Vinayak's team evaluated multiple vendors. They needed a partner who could move fast, understand a niche product (medical exam prep is not generic e-learning), and build something that could survive exam-season traffic without falling over. More importantly, they needed a team that would stay — not just ship an MVP and disappear, but grow with the product quarter after quarter, year after year. They chose VGTS.

02 · Our Approach

Ship fast, then scale — but get the architecture right the first time

2→8 Team members
7 yrs Partnership
Quarterly Release cadence
01

MVP Build & Launch

8 weeks

Scoped the MVP to a single core feature — the MCQ bank — to get to market quickly and validate the product before building out the rest

Built the Flutter mobile app (iOS and Android simultaneously from a single codebase) and the Python Django backend from scratch

Integrated Razorpay for subscription management with granular pricing — so doctors could pay for exactly what they needed, not expensive bundles

Built a React-based admin panel giving the Surgtest content team full autonomy to manage questions, answers, explanations, and topic taxonomy without developer support

Shipped to the App Store and Play Store in 8 weeks from kickoff

02

Video & Scale Infrastructure

Months 3–6

Added video lecture delivery via Vimeo's streaming API — required multiple iterations through different video players before finding a configuration that performed reliably across varying mobile network conditions in India

Built a real-time grand mock test leaderboard enabling students to benchmark their rank live against thousands of peers nationwide

Engineered AWS auto-scaling to handle exam-season traffic spikes — when daily active users could surge 5–10× overnight without warning

Expanded content coverage from General Surgery to 20+ super-speciality streams, each with dedicated MCQ banks and video lecture series

03

AI Integration & UX Overhaul

2025

Launched an AI study companion alongside a full UX redesign in a single coordinated release

The AI companion adapts to each student's performance history, surfacing weak areas and recommending next study sessions

Average study session length doubled from 2 hours to 4 hours post-launch — the clearest signal that the product had become genuinely useful, not just functional

VGTS team grew from 2 engineers at launch to 8 people supporting Surgtest across development, infrastructure, and support

04

Continuous Evolution & Automation

Ongoing — every quarter

Every quarter, we ship new features that benefit four stakeholders simultaneously — end users (students), the marketing team, operations, and the business owners

Built a WhatsApp-based support and commerce system — students can get doubt resolution, receive exam notifications, and purchase subscription packages directly inside WhatsApp without ever opening the app

Implemented end-to-end automation across content publishing, subscription lifecycle management, exam scheduling, and performance report generation — reducing manual operations by over 80%

Continuous iteration based on student feedback loops, exam pattern changes, and competitive analysis — the platform evolves with every NEET SS exam cycle

03 · The Solution

A platform built for doctors who study between surgeries

MCQ Filter Screen

Deep filtering by topic, subtopic, difficulty

Analytics Dashboard

Chapter-wise and peer-rank analytics

AI Companion Screen

Personalised study recommendations

Adaptive MCQ engine with deep filtering

Students filter by topic, subtopic, difficulty, and past performance. The system reshuffles questions to target weak areas and maximise recall. Both Test Mode and Practice Mode available for every bank.

Session-length-optimised for clinical life

Built for 15-minute sessions between hospital rounds or surgeries. Offline-capable, fast-loading, and designed to resume exactly where a student left off.

Live peer benchmarking & performance analytics

Chapter-wise, test-wise, and peer-rank analytics let students see not just how they're doing — but how they compare against thousands of other NEET SS aspirants preparing simultaneously.

AI study companion

Analyses each student's full performance history and guides the next study session — surfacing gaps, recommending content, and reducing the time students spend figuring out what to study next.

No-bundle pricing model

Doctors buy exactly what they need: any MCQ bank, video series, or mock test by speciality and duration. No expensive all-in-one packages. Priced at 3× less than the nearest competitor.

WhatsApp-native support & commerce

Students get doubt resolution, exam notifications, and can purchase subscription packages directly inside WhatsApp — no app download required for the first touchpoint. Automated flows handle FAQs, payment links, and renewal reminders.

Surgtest is priced at approximately one-third the cost of the nearest competitor — yet consistently outranks them on user ratings and rank holder outcomes. In a price-sensitive market, the right combination of quality and value builds a defensible moat.
